EVALUACIÓN DE LA AUTOFLUORESCENCIA DE LA PIEL COMO MARCADOR PRONÓSTICO DE SALUD CARDIOVASCULAR EN PA

La dislipidemia de forma general consta de un grupo de enfermedades metabólicas en relación a la variación de colesterol y triglicéridos, con alto potencial de enfermedad cardiovascular particularmente enfermedad arterial coronaria y la posibilidad de afectación orgánica sobre otros órganos, tal es el caso de la enfermedad arterial periférica o de la enfermedad hepática. Se conocen múltiples sistemas de clasificación para la dislipidemia, pero en el ámbito cardiovascular prevalece si estas generan ateroesclerosis coronaria o no. Dentro del espectro de las dislipidemias con impacto en la enfermedad cardiovascular se acentúa a la elevación de la lipoproteína de baja densidad, que es el factor de riesgo modificable más ampliamente estudiado asociado en la enfermedad cardiovascular aterosclerótica. Teniendo como protagonista a la LDL consideramos dos condiciones derivadas de su metabolismo; la dislipidemia familiar, es una enfermedad de origen genético con elevación sostenida de LDL con alta incidencia de eventos cardiovasculares a temprana edad. En segundo término, pacientes de alto riesgo son aquellos que presentaron un evento cardiovascular en ausencia de dislipidemia familiar [debido a las características clínicas] y que a pesar del tratamiento médico optimizado [estatinas de alta eficacia y ezetimibe] para el control de lípidos posterior a un infarto de miocardio no alcanzan las metas necesarias para reducir el riesgo de otro evento cardiovascular. Los pacientes con DLF y post infarto se consideran sujetos de alto riesgo cardiovascular y el conjugado con colesterol LDL fuera de meta ensombrece el pronóstico de estos pacientes. Desde el estudio de la ateroesclerosis, coronaria y periférica, se estableció la fuerte asociación entre dislipidemia [particularmente el aumento del nivel de colesterol LDL] y la progresión de dicha enfermedad, desde mecanismos clásicos hasta nuevos alcances en la evolución de la misma. El uso de inhibidores de PCSK9 como tratamiento en paciente con dislipidemia familiar y con enfermedad cardiovascular revoluciono en la última década los desenlaces de este grupo de pacientes. El seguimiento se establece principalmente por las cifras de colesterol LDL que se ajustan a las metas establecidas en consenso para cada caso, además de vigilancia de otros parámetros bioquímicos y de gabinete. Contemporáneamente dichos estudios abarcan el protocolo de seguimiento, pero existe una brecha entre la evaluación mínimamente invasiva de tales pacientes sometidos al tratamiento; buscando una herramienta que aporte información sobre su evolución, sea repetible y brinde a cada médico al frente de estos pacientes información que impacte sobre la salud cardiovascular. El propósito de esta investigación es comparar la autofluorescencia de la piel como un marcador adicional de salud cardiovascular en pacientes con dislipidemia familiar y dislipidemia sin control en pacientes tratados con inhibidores de PCSK9.
